A 35 year old gentleman with background type 1 diabetes (left eye blindness secondary to proliferative retinopathy); end stage renal failure (functioning live unrelated, high immunological risk, right iliac fossa renal transplant implanted 2 years ago); morbid obesity with obstructive sleep apnoea on bilevel positive airway pressure (BiPAP); coronary artery bypass 5 years ago (including LIMA Graft); and intracranial haemorrhage 12 years ago (resolved with conservative management), presented with exertional breathlessness (New York Heart Association NYHA class III) and chest pain over the last 3 months.
